Chronic Headaches and Migraine: Understanding the Difference and Finding Relief

Almost everyone experiences a headache occasionally — but when headaches become frequent, severe, or disruptive to daily life, it's worth understanding what's happening and exploring options that go beyond over-the-counter relief.

Headache vs. Migraine

Not all headaches are the same. A tension-type headache often feels like a dull, constant pressure around the head, while a migraine is typically more intense, often one-sided, and can come with nausea, sensitivity to light and sound, and sometimes visual disturbances known as "aura." Recognizing the pattern is the first step toward effective treatment.

What Causes Chronic Headaches?

Triggers vary widely from person to person and can include stress, poor sleep, dehydration, certain foods, hormonal changes, and screen time. In some cases, headaches can also be related to underlying issues such as sinus problems, vision strain, or — less commonly — other neurological conditions that require evaluation.

When Headaches Need Medical Attention

While most headaches are not dangerous, certain warning signs warrant prompt evaluation: a sudden, severe headache unlike any before, headaches accompanied by fever, stiff neck, vision changes, weakness, or confusion, or a significant change in headache pattern, especially after age 50.

Diagnosis

Diagnosis is largely based on a detailed history of symptoms, frequency, and triggers. In certain cases, imaging such as an MRI may be recommended to rule out other causes, particularly when headache patterns change suddenly or are accompanied by other neurological symptoms.

Treatment Options

Management often combines identifying and avoiding triggers, lifestyle adjustments, and medication — both for treating acute attacks and, for frequent migraines, preventive treatment taken regularly to reduce how often they occur. For chronic, treatment-resistant migraine, additional options such as targeted injections may also be considered.
